In the Hazard Register, you can link one or more risks to a hazard to describe potential events or outcomes related to it.
To create a new Hazard Risk:
- Go to Menu > Safety > Hazard and Risk Register
- In the Hazard Browser pane, single-click on the Hazard you want to add a risk to.
The right side of the page should refresh with that Hazard's individual tabs ("Hazard", "Hazard Links", "Hazard Documents", "Hazard Actions") and potentially existing risks that have been added earlier.
- In the top right, click on the New Risk button.
- In the Create New Risk dialog box, complete the following fields:
- Risk Name – Enter a name for the risk.
- Likelihood – Select the likelihood of the risk occurring.
- Consequence – Select the potential consequence if the risk occurs.
- Risk Index – This is auto-calculated based on the selected likelihood and consequence, according to your organisation’s risk matrix.
- Adequacy of Existing Controls – Select the option that best describes how effective current control measures are.
- Risk Priority – Choose the priority level for this risk.
- Risk Owners – Assign one or more personnel responsible for managing this risk.

Risk Equipment – Click [Add/Remove Equipment] to add any relevant equipment to this risk profile. If no equipment is listed, the full list needs to be created via the Hazard and Risk Register Setup.
- Click Create to finalise adding the risk.
Risk Controls
Controls are measures or actions that help to reduce the likelihood or impact of a risk. They can be tracked and documented to ensure the organisation understands what is in place to manage risks effectively.
To add Risk Controls:
- Select the Risk Controls tab.
- Click Edit.
- Click +Add Control.
- Enter the control measure's name in the text box.
- Select the control status from the dropdown (In-Effect, Proposed).
- Click the checkmark icon to confirm.
- Click Done when you have completed making changes to this page.
Risk Review
A Risk Review allows organisations to periodically reassess the current status of a risk. This includes evaluating whether the likelihood or consequence has changed.
We recommend adding an initial Risk Review and then scheduling the next ones with the desired date or frequency.
To schedule Risk Reviews:
- Go to the Risk Review tab.
- In Next Review Date Options, click the Edit button.
- Choose the frequency.
- Click Save.
When the review is due, the Risk Owner will be notified.
